Athletic Physicals – Click Here For Info

Athletic physicals for rising 5th – 12th grade students are scheduled for Thursday, May 11th in the lower elementary building.
Upcoming 5th – 8th graders will need to come at 5:00. Upcoming 9th – 12th graders will need to come at 5:20.
Siblings of seniors will be allowed to go first so they may attend the Senior Honors Night Banquet.
The required forms are available on Bloomz or in the HS office.
Athletic physical forms and Archbold consent forms must be completely filled out and signed by a parent in order to have the physical done.
